Tirzepatide is a long-acting synthetic peptide and a first-in-class dual-receptor agonist. It is an FDA-approved medication widely used for the treatment of type 2 diabetes and chronic weight management.
Its mechanism of action involves the simultaneous activation of two key incretin hormone receptors: the Glucose-dependent insulinotropic polypeptide (GIP) receptor and the Glucagon-like peptide-1 (GLP-1) receptor. This dual agonism creates a synergistic effect, leading to potent improvements in glycemic control, significant appetite suppression, and substantial weight loss that has been shown in clinical trials to exceed that of therapies targeting the GLP-1 receptor alone. Its structure includes a fatty acid moiety that facilitates binding to serum albumin, which protects the molecule from degradation and results in a long half-life of about five days, allowing for once-weekly subcutaneous administration.
Tirzepatide is marketed under the brand names Mounjaro® for type 2 diabetes and Zepbound™ for chronic weight management. It represented a significant advancement in metabolic medicine as the first approved dual GIP/GLP-1 agonist and serves as a key benchmark for multi-agonist therapies.
